EXPRESS ASSUMPTION OF RISK

I, (Participant's name) understand and agree that neither Undersea Explorer nor crew may be held liable in any way for any occurrence
in connection with this diving program which may result in injury, or damages to me or my
family.
In consideration of being allowed to undertake these dives, I HEREBY PERSONALLY ASSUME ALL RISKS in connection with the program, far any harm, damage, injury or death
which occur while I am participating in the program, including without limitation, all risks
connected herewith, whether foreseen or unforeseen.
I, as a diving participant, recognize the inherent risk involved in this adventure sport activity,
including the effects of environmental conditions, nitrogen, baratrauma, and marine life.
I accept these risks as my own and it is my intention by this instrument to EXEMPT AND
RELEASE UNDERSEA EXPLORER AND IT'S CREW from all liabilities.
I further acknowledge that having been advised of the risks associated with scuba diving, I wish to undertake an adventure dive trip with UNDERSEA EXPLORER and hereby release and hold
harmless UNDERSEA EXPLORER AND IT'S CREW from any suit, demand or claim arising as a consequence of death or injury received by me during my participation in said dive trip.
I further state that I am of lawful age and legally competent to sign this affirmation and
release, or that I have acquired the written consent of my parents or guardians; that I
UNDERSTAND THE TERMS HEREIN ARE CONTRACTUAL and not a mere recital, and that I have signed this document of my own free will. I further acknowledge that this
document may be relied upon in any proceedings instituted in any Court by me or my heirs,
executors and assigns.
The information that I have provided is accurate to the best of my knowledge. I verify that I
am physically fit, not under the influence of alcohol or drugs at this time and sufficiently
qualified to participate in this event.
